Incluso: cuando quieres añadir algo.
ejemplo: tienes dinero para comprar un auto, incluso, dos si quieres.
Ni siquiera: cuando no quieres hacer o no tienes algo que es muy básico o fácil de hacer o tener.
ejemplo: Tienes que ir a la universidad y ni siquiera te has levantado de la cama.
o ni si quieras tienes los libros.

Ni siquiera do emphasis in negative sentence.
_ Ni siquiera puede mover el brazo. / He cannot move his arm.
Incluso is used to surprise about unexpected situations.
_ Levanto la mesa, ¡incluso con el brazo roto!/ He lifted up the table, even with his broken arm.
Also sometimes used incluso to hypothetical situations
_ Voy a ir a trabajar, incluso con el brazo roto./ I'm going to work, even my arm was broken.
