"Check on" makes to go see, to examine.
"I'm going to check on the kids and make sure they're okay."
"Check out" means to officially leave, or it can mean to see/investigate also.
"I'm going to check out of (officially leave) the hotel, and then get breakfast."

To “check on” someone is to see how they’re doing and if they’re okay.
To “check someone out” is to look at someone in a flirtatious way, as if you’re examine their (most likely attractive) appearance. It means you’re attracted to them and you can’t look away from them, so you keep “checking them out.”
