This question is just asking you how you have been feeling or how your life has been going since they last seen you and is usually used between people who know each other but have not seen each other for a while.
"How have you been?" can be answered many different ways all depending on how you feel. "I've been good", "Hanging in there", "Well", "Oh, you know, life's been hectic" are some examples of how to answer it but there's no limit of how to answer it. It's basically just "How are you?" but in the past tense.
If "how are you" is present tense (how you currently feel), "how have you been" is past tense (how you have been feeling in the days before the present one)
